Antwi-Adjei began playing for his hometown club, Fortuna Hagen, in 1999. He then featured for various youth sides at Hasper SV, MSV Duisburg, Eintracht Dortmund and Westfalia Herne, making his debut in the Oberliga Westphalia for the latter in 2013. Antwi-Adjei then moved to TSG Sprockhövel, before signing for SC Paderborn 07. While at Paderborn, he was part of the squad that won promotion twice in successive years, taking them from the 3. Liga to the Bundesliga. He also earned three caps for Ghana during this time due to his strong performances. Antwi-Adjei returned to the Ruhr ahead of the 2021/22 season, signing for newly-promoted Bundesliga side VfL Bochum. While at VfL, the winger helped the club secure safety in three straight seasons. He has featured 113 times in the Bundesliga to date, along with 62 matches in the second division.
